You will be asked to acknowledge you have read and understood the following terms of service on your Initial Consult form.
You understand that your proposed treatment plan and aftercare advice will be discussed with you.
You understand that Myotherapy uses a range of modalities including deep massage, dry needling, cupping, heat and cold therapy and movement therapy.
You understand there may be tenderness for several days following a Myotherapy treatment and that this is normal.
To your knowledge you have no health concerns or uncontrolled medical condition that would affect your ability to receive Myotherapy care.
You understand you may be charged a fee of $50.00 if you give less than 24 hours notice of cancellation and the spot can’t be filled. You’re welcome to send a friend/family member in your place.
You understand it’s a policy of Waverley Myotherapy Clinic to follow up on your progress to ensure high quality care.
You understand Myotherapy is bound to the general health services code of conduct which is displayed in reception. If you would like more information on any of these policies it will be provided to you or you may click here to view the code online.
You understand that if your booked Practitioner is unwell or can’t make their shift that another Practitioner may fill in.
Full Cancellation / No Show Policy
We require a minimum of 24 hours’ notice should you be unable to attend your appointment. If you are unable to provide 24 hours’ notice for cancellation/reschedule or if you don’t turn up for your appointment you may be required to pay a fee of $50.00.
We are a very busy clinic and often our practitioners have a waitlist. The more notice you give the higher the likelihood of us being able to fill the spot.
Even better to ensure the spot is filled you could send a friend or family member in your place, even if they are a new customer, we would be more than happy to accommodate them.
We understand that sometimes life gets crazy and that things pop up last minute. That’s why the first time it happens we are happy to waive this fee. You will be sent an email and/or a text reminding you of our policy and confirming the waiver.
